How to create and use WhatsApp Flow Forms for data collection in WhatChimp

Updated 16 June 2026

WhatsApp Flow Forms let you collect information from customers using native forms right inside WhatsApp — no external links needed. Customers tap a button, fill out the form, and their responses go directly to you.

How to Create a WhatsApp Flow Form

Go to Bot Manager → WhatsApp Flows and click Create.

Fill in the form settings:

Flow Name — A unique name for your form (e.g., "Request Demo").

Flow Category — Select the form type from the dropdown (e.g., Lead Generation).

Screen Unique Name — An identifier for the screen. Must have no spaces — spaces will cause errors.

Form Title — The heading customers see at the top of the form.

Reply Sent After Form Submission — Select a bot reply to send automatically after the form is submitted (e.g., a "Thank you, our team will get in touch" message). If you haven't created one yet, go to Bot Manager → Bot Reply → Create and set one up first.

HTTP API — Optionally connect an external API to send form data to your CRM or other systems. See External HTTP API Integration in Bot Flow for setup details.

Add Form Elements

Drag and drop elements onto the form board to build your form. Double-click any element to edit it — you can set the label, mark it as required, and assign it to a custom field for tracking.

ElementWhat it does
HeadingAdds a title or section label to the form
Text FieldSingle-line text input (name, email, etc.)
Text AreaMulti-line text input (messages, notes, feedback)
Radio GroupSelect one option from a list (e.g., gender, plan type)
Checkbox GroupSelect multiple options from a list
Date FieldDate picker input
Submit ButtonSubmits the form and saves all responses
Tip: Assign each form element to a custom field so responses are saved to the subscriber's profile and tracked in analytics. Create custom fields in Subscriber Manager if needed.

Save your form when done. Make sure the screen name has no spaces — the save will fail otherwise.

Publish the Form

Click Actions → Publish to make the form available for use in your chatbot.

Other actions available from this menu: Preview Flow (see how the form looks), Add Webhook (send data to external apps), Report Data (view collected responses), Download Data (export as spreadsheet), and Edit / Delete.

Connect the Form to a Chatbot

Once published, you need a chatbot to send the form to customers.

Go to Bot Manager → Bot Reply → Create. Set up a Start Bot Flow component, add a WhatsApp Flows component, select your published form, and give the button a label (e.g., "Click here to request demo"). Save the bot flow.

Set a keyword trigger — assign a keyword (e.g., "demo flow") so the form is sent automatically when a customer messages that word.

Test and Track Results

Test the flow by sending the trigger keyword to your WhatsApp number. The bot sends a CTA button — tapping it opens the form inside WhatsApp.

View collected data — go back to WhatsApp Flows, click Actions → Report Data to see all form submissions. Click Download Data to export the responses as a spreadsheet, or use Add Webhook to send data to your CRM automatically.
